Take control by creating a personal health record (PHR) to collect, track, and share information about your health or the health of someone in your care. Personal health records are proven to improve the quality of your care, reduce healthcare costs, ensure you have completed and updated information available during an emergency, and help keep you on track for appointments and vaccinations.
