The Five Wishes: An Advanced Healthcare Directive
In this workshop, participants will have the opportunity to complete The Five Wishes, which is an advanced healthcare directive. This event allows people to make conscious decisions regarding their end-of-life choices. We plan most major events in our lives but fewer than one-third of US residents have completed an advanced healthcare directive. By joining us you will be giving a gift to the people you love.
We are offering the workshop in two formats, in-person on Sunday, August 18 OR virtually on Zoom on Thursday, August 22.
IN-PERSON WORKSHOP: The price is $25 for the in-person workshop on Sunday, August 18 from 1 pm - 3 pm and includes a printed booklet you will use during the class. You can register online or at the door.
VIRTUAL WORKSHOP: If you are attending virtually on Thursday, August 22, from 6:30 pm - 8:30 pm, registration is $20. You must register online, and you will need the digital booklet to participate. It is available for $15 at https://store.fivewishes.org. You will receive the Zoom link upon registering.
